JOB DESCRIPTIONTitle of Position: Substance Use CounselorEmployed by: Upper Manhattan Mental Health Center Job Type: Full-time, Monday -FridayDescription of Duties and Responsibilities: Upper Manhattan Mental Health Center (UMMHC), a comprehensive behavioral health clinic that has served Harlem and Washington Heights for over 30 years, is seeking a Substance Abuse Counselor for our newly funded Certified Community Behavioral Health Clinic (CCBHC) program. CCBHC is a transformative initiative, funded by the Substance Abuse and Mental Health Services Administration (SAMHSA), that is designed to integrate health care services within our agency. The counselor provides assessment and treatment as well as education and training to clients and their families. The appropriate candidate has experience working with clients with co-occurring severe mental illness. Specifically, the substance abuse counselor will be responsible for:Conducting psychosocial evaluations vocational assessments and developing treatment plansMeeting with clients on a regular basis, documenting all progress in an electronic health record (EHR) and providing ongoing counseling Providing referrals to other providers within the agency or to outside agencies as needed and monitoring clients' compliance with referralsWorking with nursing staff to monitor clients' substance useabuse through urine toxicologyProviding crisis intervention as neededProviding psychoeducation to clients and their family membersLeading group counseling sessionsCoordinating client care with other behavioral health care providers including psychiatrists, behavioral health therapists, case managers and primary and specialty care providers via (virtual) daily team meetingsOther responsibilities as needed Reports to: Program DirectorSalary: $55,000 - $65,000RequirementsQualifications for Position:•Bilingual Spanish preferred•CASAC License or LMCSW or LMHC•1-4 years of experience•Must be comfort able utilizing telemedicine platforms and video conferencing systemsPersonal Qualities: Looking for a team player and strong communicator
