Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.We are Now Hiring a Qualified Intellectual Disability Professional in Brooklyn, NY to provide person-centered care tailored to meet the needs of individuals with intellectual and developmental disabilities. The Qualified Intellectual Disability Professional assists the Executive Director in developing and maintaining individual service plans that meet the needs of people with disabilities and state and area program guidelines.Starting pay is $23.00hour ResponsibilitiesDevelop individual service plans (ISP)Person Centered Plans (PCP)Coordinate admission and discharge activities of participantsAct as a liaison for area programs with clinical issuesProvide support and supervision to Sr. Community Support Staff regarding compliance and clinical guidelinesPrepare monthly goal progress notes and billing justification formsMaintain and analyze all incidentaccident reportsFacilitate selection of clinical consultants, and monitor their activityCoordinate behavioral intervention plans, to include routine and emergency servicesMonitor medication procedures, administration, and documentationEvaluate participant satisfactionMonitor staff compliance with plan implementation and licensing complianceEvaluate staff performanceEmployee must cooperate with the licensee and department staff in any inspection, inquiry or investigationAdditional tasks and responsibilities may be assignedMinimum RequirementsBachelor's degree and two years' experience in implementing community-based support service for people with disabilitiesValid driver's license and good driving recordQIDP qualifiedExcellent organizational skills, and the ability to set priorities and follow through under time constraintsAbility to effectively and professionally communicate with program participants, and other agencies, staff and members of the communityTraining in Gentle Teaching methodsAbility to respond to crisis situations, exercise professional judgement, and use common senseKnowledge of required state operating and reporting guidelines under OPWDDWorking ConditionsWork in residential programs, day programs, and in the communitySchedule may change due to business needs and may include evening and weekend hoursMay be required to be on-call in cooperation with other management staffFrequent lifting, stretching, and other physical exertion may be requiredMay be required to transport individuals utilizing your own vehicle or company provided vehiclesMay be required to lift or move 25+ poundsMay assist with wheelchair transfer of non-ambulatory individualsMay be exposed to various medical conditions and communicable diseasesWhile performing the duties of the job, employee travels by automobile, utilizing own vehicle, and is exposed to changing weather conditions.Why Community Options?
