The Bedford-Stuyvesant Family Health Center (BSFHC) is a 501C3 Federally Qualified Health Center (FQHC) that serves all of the primary health care needs of families in the heart of North and Central Brooklyn for over 40 years. Our mission is to provide the most professional, courteous and highest quality health care, with dignity, to those we serve, especially the undeserved population, without regard for ability to pay. The health center's Communities of Color (CoC) initiative aims to increase awareness in the community about health-related issues to improve the quality of care and delivery of services targeted towards women of color. The program will achieve this goal by linking community members to program services. The Program Coordinator implements the CoC initiative's health education and patient navigation services, with emphasis on sexual and reproductive health among women of color. Working with the Senior Program Manager, this role combines outreach activities with comprehensive support services to enhance healthcare accessibility and promote health equity.Bilingual Spanish preferred. Required field about 30% of the time and take mass transit to travel throughout New York City.Grant-funded: Contingent upon continuation of grant funding from year to year.Duties and Responsibilities include but not limited to:Develop partnerships at the community-level to identify women who are underserved, uninsured, out-of-care, especially people living with HIV (PLWH) as well as those in need of access to behavioral health and chronic disease management.Conduct outreach activities at various community organizations and healthcare settings to raise awareness on insurance coverage and health-related issues providing information on services offered by BSFHC through supporting programs.Implement Sister to Sister program curriculum and facilitate health education sessions focusing on sexual and reproductive health for women of color.Coordinate patient navigation services including appointment scheduling, insurance enrollment assistance, and pharmacy coordination to ensure continuity of care.Manage and track referrals to healthcare services, ensuring successful linkage to care and follow-up support.Develop and maintain partnerships with community organizations to expand program reach and enhance service delivery.Document all client interactions, service delivery, and program outcomes in appropriate tracking systems for reporting purposes.Collaborate with clinical teams to ensure smooth integration of navigation services with medical care.Provide HIV navigation services including testing coordination, linkage to care, and treatment adherence support.Support program evaluation by collecting and analyzing data on service delivery and client outcomes.Perform additional duties assigned by the Senior Program Manager.Knowledge, Skills & Abilities:Strong understanding of community health disparities and cultural competency in healthcare delivery.Excellent interpersonal skills and ability to work effectively with diverse populations.Proficiency in case management and patient navigation principles.Knowledge of healthcare systems, insurance programs, and community resources.Strong organizational and documentation skills.Experience with sexual health education and HIV services preferred.Education Required Bachelor's degree in public health, Social Work, or related field required.Other Related SkillsExperience: Maintain HIPPA patientemployee confidentialityKnowledge of EMR systems a mustExcellent interpersonal skills must demonstrate professionalism, courteous and respectful attitude in dealing with patients, team members, families and significant othersBenefits Overview: We offer attractive compensation with comprehensive benefits including: Medical, 401k Retirement Plan with discretionary Match, Free Life Insurance and Long-Term Disability, Loan forgiveness program, Transportation Plan, Generous Paid Vacations and Holidays.
