Isaacs Neighborhood Center (Isaacs Center) is a non-profit, multi-service organization focusing primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville and seek to deliver programs that are impactful, innovative, and intergenerational.Position Summary: The Program Coordinator (Taft) is responsible for managing the day-to-day operations of the Community Telehealth Project. The Program Coordinator (Taft) will also be providing case management services to older adults and leading program activities. Other duties will include conducting outreach to promote programs to identify and enroll new participants and working with partner agencies to meet deliverables. Provision of services will be in accordance with Isaacs Center policies, contractual guidelines, and standards. Schedule: 35 hours per week. Salary: $45,940- $57,425Roles, Responsibilities, and Essential DutiesManage the day-to-day operations of the Community Telehealth Project which include: developing creative methods to promote the program, identifying and enrolling participants, acting as a liaison with funders and community partnersProvide case management services to older adults residing within the program catchment area and surrounding community which include: applying for government benefits and entitlements, advocating for the clients' medical, mental health, housing-related and other needs, and coordination of carePrepare reports and documentation as stipulated by contractual and agency standards and proceduresAssist with NDA Senior Service Program and provide staff coverage when neededAssist with program planning which include: developing, implementing, and promoting educationalrecreational groups, and leading programsDevelop and foster relationships with community and government agenciesOther duties as assignedQualifications: The successful candidate for the role of Aging Services Coordinator should have:Demonstrated experience in program startupAbility to multitask and manage priorities, developing and leading programs, and providing thoughtful leadership. Excellent interpersonal, communication, time management, organizational, and teaming skills are essential.
