Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Visit us at DescriptionPurpose of Position:The Assistant Addiction Counselor is a vital member of the Interdisciplinary team and responsible to assess all clients in the SBIRTED Addiction Leads & CATCH Service referred andor consulted for alcohol andor substance abuse issues for treatment readiness and referralDuties & Responsibilities:•Conducts initial screenings of patients and performs ongoing toxicology screening to identify possible substance use disorders and prepares accurate screening reports.•Provides basic orientation to new patients to familiarize them with the program's rules, goals, procedures, services and expectations over the course of treatment.•Demonstrates understanding and knowledge of principles and philosophy of prevention, treatment, and recovery and how they relate to successful achievement of patient treatment goals and objectives through coordination and provision of case and care management services and activities that integrate supportive services, agency, professional andor community resources in a planned framework of action, which address a patient's mental, physical, andor behavioral needs.•Provides basic crisis support by responding to and following-through in crisis situations, and seeks additional assistance in the management of crisis situations, when needed.•Conducts site visits andor assists with outreach activities and advocacy designed to educate staff and communities on substance abuse and addiction, prevention, availability of services andor other related matters.•Discusses financial responsibility with patients in the case of outstanding bills, and provides money management assistance.•Assists with intake interviews of patients admitted to SUD treatment, and in the development of psycho-social histories to determine and isolate specific patient problems. Prepares accurate intake and assessment reports.•Co-facilitates groups, provides psycho-education, and conducts skills training classes, as directed.•Assists with liaison functions and in making referrals to job training, education programs, social service agencies, housing and other necessary services, in accordance with the program's policies and procedures; collaborates with referral sources to facilitate effective and timely patient care management; may make field visits, as needed.•Participates in CATCHSBIRTED LeadsRecovery Center Team and unit interdisciplinary team rounds and provides updated information about the patients to the team;•Provides information related to in CATCHSBIRT inpatient consultation services to patients and next of kin as needed or as required by the interdisciplinary team.•Provide education to other Hospital Personnel about CATCHSBIRTED LeadsRecovery Center and other available treatment facilities;•Negotiates and provides direction to patients regarding contingency contracting (appointment scheduling, screening mechanisms, etc.).•Provides follow-up and aftercare, as necessary.•Completes monthly statistics•Can be assigned to other designated areas in the Substance Use Division service units as needed or when deemed appropriate by Supervisor andor designee;•Responds and participates in external and internal disasters when indicated or assigned by Supervisor.•Assists with the preparation of case records and reports (e.g., New York State Office of Alcoholism and Substance Abuse Services (OASAS) reports, group reports) and maintains confidential patient records, in accordance with appropriate policies and procedures.•Participates andor completes internalexternal quality assuranceperformance improvement (QAPI) activities, programs, training and related mandated health care events, as required.•Complies with NYC Health + Hospitals policies and procedures, and applicable local, state and national requirements and guidelines.•In non-OASAS-certified treatment programs, may assist in counseling patients admitted to SUD treatment in one-on-one or in group sessions andor assist in development of treatment plans. In OASAS-certified treatment programs, may only assist in counseling patients with a valid CASAC-Trainee certification and under supervision.•Performs duties as assigned by Supervisordesignee.Minimum QualificationsAny individual working in an OASAS-Certified Treatment Program or newly-hired into the title of Assistant Addiction Counselor in a NYC Health + Hospitals health care setting must have proof of completion of OASAS "Supporting Recovery with Medications for Addiction Treatment (MAT)" training and "Introductory Training in the Addictions" training.1. Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) - Trainee Certification issued by the NYS Office of Addiction Services and Supports (OASAS); or2. Bachelor's degree from an accredited college or university including or supplemented by twenty-four credits in Psychology, Sociology, Social Work or a related field; or3. High School diploma or its educational equivalent and two (2) years of full-time paid satisfactory experience performing case management, care coordination, and related supportive functions in a substance use disorder treatment program; or4. Satisfactory equivalent of education, training, and experience.
