A healthcare network in New York is currently seeking a licensed Nurse Practitioner (NP) to join their hospice team in the Bronx. About the Opportunity: Schedule: Monday to Friday Hours: Standard business Specialty: Hospice Setting: Field and facility Patient Population: Hospice Responsibilities: The Nurse Practitioner (NP) will: Coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed Provide in-person and telehealth visits, as needed Provide support and guidance to patient's family members and caregivers, as necessary Other duties, as needed Qualifications: Bachelor's andor Master's Degree NYS Nurse Practitioner (NP) license Desired Skills: Previous experience in hospice and palliative care
