Nexus-FACTS seeks a hybrid remote Youth & Family Skills Worker for our Youth Care Transition (YCT) program. The YCT is a new opportunity to provide intensive support to youth and their caregivers as they transition out of a Psychiatric Residential Treatment Facility (PRTF), Child and Adolescent Behavioral Health hospital (CABHH), or an emergency room hospital setting. YCT program provides placement coordination services and intensive therapeutic services with the youth and their caregivers through a multi-disciplinary teamed approach.
