Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We work every day for a fair and just society where all people have the opportunity to make choices that lead to better lives for themselves and their families.Purpose of Position: To deliver effective case management services using a strengths-based, client-centered approach to promote optimal well-being and maintain stable housingSchedule: Monday through Friday 9:00 am to 5:00 pmSalary Range: $46,000 - $52,000Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate's number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.Roles, Responsibilities, and Essential DutiesServe as case manager for formerly homeless adults with mental health, substance use and other co-occurring disordersPerform intake, assessment, service planning, advocacy, referral and escort servicesProvide case management; identify short and long-term treatment goals and help clients achieve them, including linkages to community programs such as medical, psychiatric and drug treatment services.Utilize evidence-based practices such as person- centered planning and motivational interviewing.Maintain case records with documentation of services provided and outcomes, according to specific program procedures. Responsible for case notes and service plans using an electronic databaseAssist clients in receiving and maintaining entitlements Track and Monitor Covid cases, and Hospital visitsAccompany clients on visits to doctors, entitlement offices or other community services as neededServe as liaison between other service providers and community resourcesEncourage engagement and participation in services and activitiesParticipate in weekly staff meetings, case conferences, training programs and supervision meetingsAssist with the design, planning, and implementation of group activities and eventsQualificationsEducational RequirementsBachelor's Degree requiredExperience working with homeless adults preferredExperience working with individuals diagnosed with severe and persistent mental illness, substance abuse andor co-occurring disorders preferredPrior case management experience preferredBilingual (English and Spanish) preferredSkills, Knowledge and AbilitiesUnderstanding of housing first modelUnderstanding of severe and persistent mental illnessUnderstanding of harm reduction and trauma informed careStrong interpersonal skillsStrong written and verbal skills Team playerComputer Skills: To perform this job successfully, an individual should be:Proficient in Microsoft Outlook and ExcelAble to use or learn computer databases such as AWARDS, MAVEN, PaycomPhysical Requirements Walking and traveling to escort clients to various appointments throughout the cityOccasional visits to hospital emergency roomsWork EnvironmentMostly indoor officeRegular home visits to client living spaces requiredEmployee Benefits:MedicalDentalVisionLife InsuranceCommuter benefitsEmployee Assistance Program (EAP)Flexible Spending Accounts403B Thrift Retirement Plan12 Annual Sick Days12 Agency Holidays20 Days or 4 Weeks of Paid Time off and an additional week (5) days after 5 years of employmentTotal of more than 6 weeks (a month and half) of paid time offWe are committed to building an inclusive workplace community grounded in respect for differences.
