Job Title: Care Coordinator - full-timeLocation: Glen Cove, NY (Travel throughout Nassau County required)Hourly Rate: $26.50 per hour; 35 hours per week scheduleSchedule: Monday - Friday (some flexibility needed to meet caseload needs)Position Overview:The Charles Evans Center is seeking a compassionate and skilled Care Coordinator to join our expanding care coordination team. In this role, you will be a vital member of an interdisciplinary team dedicated to providing care coordination for adults diagnosed with severe mental illness, multiple medical co-morbidities, andor co-occurring substance abuse disorders.Key Responsibilities:Advocate for and support clients in navigating the healthcare system.Engage with community agencies and healthcare providers to ensure clients have access to necessary services.Facilitate wellness self-management and reduce emergency room visits and hospitalizations.Provide expert support and psychosocialsubstance abuse interventions.Maintain a focus on client outcomes and best practices.Qualifications:Associate's Degree with two (2) years of experience in the human services field; or Bachelor's Degree with one (1) year of experience in the human services field.Clean and valid NYS driver's license.Ability to travel to multiple locations during the day throughout Nassau County.Benefits:Competitive salarySupportive work environmentOpportunities for professional developmentJoin us in making a difference in the lives of individuals in our community.
