We offer a fulfilling and engaging workplace, a comprehensive and competitive wage and compensation program, and the opportunity to make a real difference in the lives of the people you touch.Responsibilities of RN MDS Assessor:Completes assessments, MDS, and care plans as assigned.Creating and disseminating care plan schedules.Responsible for the entire MDS process.Reviews MDS assessments when there is a change in a resident's condition andor quarterly.Develops Comprehensive Care Plan with the Interdisciplinary Team.Makes recommendations for charting forms staff of any changes regarding resident care and care issues.Monitors "RUG" scores for optimal reimbursement.Monitor and audit records to ensure accuracy and timelyMaintain the confidentiality of all residents, staff, and organization information. Other duties may be assigned.Requirements for RN MDS Assessor:Long Term Care experience Knowledge of PDPM and CMIWell-organized, motivated, and professionalActive NYS Registered (RN) LicenseSalary: $95,000 - $110,000
