Director of Social WorkOur home is look for a qualified, hard-working and customer service driven Social Worker with long-term care experience to join the team.Director of Social Work Responsibilities Include:Maintaining andor improving resident's ability to control everyday physical needs and mentalpsychosocial needsIdentifying individual social and emotional needsDeveloping and maintaining individualized care plansParticipating in the planning of the resident's admission, community re-entry or transfer to another facilityParticipating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the residentWork with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stayWork with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the communityQualificationsRequirements:LMSWAt least one year experience in Social Work field requiredLong term care andor sub-acute setting preferredAbility to work flexible schedulePCC experience preferredSalary Range: Up to $110,000 a year (Based on experience)An Equal Opportunity Employer
