Job Responsibilities: Work as part of an interdisciplinary team to provide comprehensive discharge planning, provide emotional support to patientsresidents and families, complete MDS and psychosocial assessments. Coordinate and facilitate discharges, care planning and completion of all documentation Facilitate team and family meetings, education on resident rights and well-being needs of the vulnerable elderly and ill population.. Skills & Requirements Sarah NeumanWestchester Division of The new Jewish Home offers a positive work environment, continuing education credits . In addition, position offers flexible working hours. Skills & Requirements MSW from an accredited School of Social Work. Valid New York State License in Social Work. Experience andor expressed interest in working with adult and geriatric population. Experience in long term care preferable , solid written, communication and organizational skillsThis is a TEMPORAY FULL TIME position | Monday to Friday | 9:00am to 5:00pm
