OverviewMDS Coordinator is responsible for coordination of MDS assessments of each resident. Participates in the overall process of providing optimum care of each resident. Provides support to clinical staff in the provision of high care quality of life for each resident. ResponsibilitiesCompletes accurate assessments, MDS, and CAAs as assigned.Monitors and audits clinical records, assures accuracy and forms PI Director of persistent issues related with non-compliant municates with physicians, discharge planners and others to process referrals, authorization for services, and capture data related to utilization.Gathers information on facility's current residents for future assessments, including physical and mental states.Ensures accurate ICD-10 coding for proper reimbursement.Maintains current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.Monitors Case Mix Index (CMI) scores, looking for potential risks andor changes that may affect Medicaid reimbursement.Reviews daily all communications received via fax, email or pletes special managed care projects; performs independent research and preparesgathers information for reports and data summaries. Assists other departments with projects as needed in the spirit of teamwork.All other duties as requested. QualificationsEducation:Bachelor's Degree in Nursing.Master's Degree highly desired.Experience:Minimum of two (2) years of experience in MDS 3.0. Long Term care experience required.Knowledge and Skills:Positive relationship building.Strong interpersonal and verbal communication anized with an ability to work independently to complete projects.Detail oriented with consistent follow-through.Excellent writing, listening and problem solving skills.Proficient with Microsoft Office suite applications such as Word and Excel.Licenses, Certifications:New York State Registered NurseObtain Resident Assessment Coordinator-Certified (RAC-CT) within one year from the date of hire.Maintain MDS RAC-CT Certification.Physical Requirements:Consistent with that necessary to work in a variety of office settings.Position requires prolonged periods of sitting, reaching, walking throughout the work day. Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
