Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job DescriptionThe Social Worker Level II provides mental health evaluation, treatment and coordination of aftercare services to adults and children residing in domestic violence shelters, in-person at designated shelter locations, assigned H+H facility location andor via Tele-Health. This social worker also provides mental health consultation and training to shelter staff, to facilitate patient access to care by H+H clinical team. The Social Worker Level II is supervised by a Social Worker Level IIISocial Worker Level V, who reports to the Associate Director of Social Work.SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:Conducts Social Work Comprehensive Assessments for all patients in accordance withdepartmental policies and procedures.Collaborates with shelter staff and other service providers to ensure proper delivery of servicesParticipates in case conferences and team meetings to review progress and status of all patientsProvides, supportive counseling, crisis intervention, therapy, grief counseling and advocacy andreferral services to shelter clients and children as appropriate.Assists clients and shelter staff in the completion applications for community resources including:Office of Victim Services claims applications, 2010E housing applications and VAWAimmigration documentation.Leads psychoeducationalsupport group for adults, children and adolescentsAssists patients with adjustment to treatment, including problems of depression, anxiety, denialandor anger; noncompliance with medical regimen.Responsible to monitor caseload from intake to discharge to ensure good patient care.Formulates and implements social work treatmentcontinuing care plans involving client significantothers and the interdisciplinary team as appropriate.Collaborates with domestic violence shelter staff to ensure comprehensive delivery of psychiatricservices to clients as needed.Ensures patient records are stored securely, confidentially, and compliant with HIPAA standards;interfaces with hospital Medical Records department as pletes all appropriate social work documentation within designated timeframes.Maintains and submits statistics on case activitiesAttends departmental, hospital, interintra agency case conferences and meetings and trainings asrequired by supervisor.Participates in and assists in the planning of community events including health fairs and domesticviolencesexual assault awareness month activities.Performs other duties assigned by supervisor as needed.Minimum QualificationsFor Assignment to Level II1. Valid license as an LMSW issued by the NYSED; or2. Valid license as an LCSW issued by the NYSED.Department PreferencesAble to utilize clinical skills to engage, assess and provide treatment to adults and children and develop appropriate aftercare plans.Able to communicate proficiently (verbally and in writing) with patients, colleagues, multidisciplinary team, managers and administration.Able to document effectively all interventions and continuing of care plans in the patient's medical record in a clear, concise and legible manner.Knowledge of the impact of psychosocial stress and trauma on mental health and well-being.
