Our children have often experienced significant levels of trauma, poverty and violence. As they grow and mature, their individual needs change. Our assessments, health counseling, and selected treatments change to meet their new needs. Our children's physical and mental health as well as their strengths must be supported so they can reach their full potential.POSITION SUMMARY:Under the supervision of the Nursing Supervisor, the LPN will be responsible in coordinating and providing the best possible care to the clients.REQUIRED QUALIFICATIONS: New York State, current CPR certification and completion of required courses in Child Abuse and Infection Control.Current Licensed Practical Nurse in New Minimum of 1 year of experience treating childrenadolescents in the nursing field. RESPONSIBILITIES:Coordinate careDevelop a comprehensive coordinated care plan based on the assessments and evaluations in conjunction with the medical home staff and treatment team. Collaborate with the Medicaid Managed Care Liaison to establish a medical home that will address current and future health care needs by identifying providers approved by the youth's Medicaid Managed Care Plan Collaborate with Medical Assistants on scheduling care - primary, dental, mental health and specialty care - and obtaining medical records.Act as a liaison between multiple providers in the provision of health services for our children in care.Document client care services by charting in patient and department records; on-going auditing of medical records to ensure compliance with regulatory agencies. Communicate with clients, court, and multidisciplinary team regarding client health information as appropriate.Participate in case conferences as needed.Support caregivers as necessary around meeting the medical dental mental health needs of children in their care. Ensure compliance with regulatory agencies' rules for children who are on psychotropic or any other medications.Liaise with hospitals if a child is hospitalized and attend any necessary conferences during hospitalizations.Client careRoutine Screenings for Substance Abuse, Developmental Health, Mental Health, and others as directed.Physical Body Checks for Signs of Abuse Reinforce Psychoeducation around matters such as routine management and training regarding chronic conditions (Asthma, Diabetes), Health Education related to Sexual Reproductive HealthExecute medical orders (tests, tasks, hospitalizations, etc.) as ordered by physicianTake vital signs, height and weight; document observations and activities Administer immunizations, hearing and vision screening. Provide first aid and emergency care as neededObtain lab specimensAdditionalEstablish a compassionate environment by providing emotional, psychological, and spiritual support to clients and families.Obtain any necessary consent.
