Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job DescriptionSUMMARY OF DUTIES AND RESPONSIBILITIES: The Lifestyle Medicine Program is designed to help patients living with chronic cardiometabolic conditions adopt healthy lifestyle changes across the pillars of nutrition, physical activity, sleep, stress management, social connections, and avoidance of risky substances. Clinical criteria for referral to this program include atherosclerotic cardiovascular disease, hypertension, prediabetes, type 2 diabetes, andor health concerns related to obesity. The Program Coordinator will oversee the day-to-day operations of the Lifestyle Medicine Program.Summary of Duties and Responsibilities: •Participate in daily huddles to ensure optimal team communication, support and problem solving.•Help develop strategies to improve performance and operations (i.e., reduce patient cycle time, improve access, decrease wait time, improve patient experience).•Coordinates key administrative initiatives including new hire placement, room assignments, and access for software systems as needed (e.g., Epic, Outlook, Microsoft Network).•Plans, develops, and may conduct trainings to ensure clerical staff consistently meet patient experience standard including tone, body language and attitude, and display caring actions even in stressful situations•Complete annual regulatory requirements within established timeframes.•Perform data entrymanagement including generating reports and maintaining a HIPAA-compliant database of program patients and their follow-up visits•Manage intake of patients to program, including outreach to prospective patients to provide overview of program and schedule initial visits•Provides administrative supervision of patient flow through the program, ensuring patients are scheduled as per program protocols, identifying patients lost to follow-up, and troubleshooting missed appointments•Assist with scheduling program appointments as needed (remote or in-person visits to physicians, nurse practitioner, dietitian, health coach, social worker).•Facilitate communication between patients and finance department on billing•Assist with requests for authorization for dietitian visits and other services.•Coordinate logistics of program activities, including group visits (video andor in-person), cooking demonstrations, and program staff meetings.•Manage communication between program staff and entities including patients, public relations, volunteers, and studentstrainees.•Performs related duties as requiredMinimum Qualifications1. A masters degree from an accredited college or university in public health education, education, publiccommunity health administration, public administration or business administration, and one year of full-time satisfactory experience in developing public health education programs, including identifying target populations, conducting needs assessments, designing educational materials, planning educational presentations or workshops, evaluating health education programs; presenting public health education programs; andor counseling in areas such as communicable diseases, substance abuse, assault, sexual abuse, andor family planning; or,2.
