Intake CoordinatorFull TimeParker Jewish Institute's Community Health Services program is seeking an experienced professional to oversee the intake function for Parker's community programs-ensuring patients accepted to the program are appropriate and evaluated timely by home care and hospice staff. This is role is key in providing both quality patient care and actively working to expand our services, as Parker's Hospice and Home Care programs continue to grow. Parker Jewish Institute's Hospice and Home Health Care offers the finest in home health services, and provides comfort, relief, and peace of mind to individuals with advanced or life-limiting illness and their families.
