We are looking for a Field Nurse for our Licensed Home Care team. Department & Position Overview:The Field Registered Nurse is an integral team member of the Freedom Care Licensed Home Care team. They assess our patients' current service needs and reassess needs for service upon significant change in health status.Every Day You Will:Conduct initial assessments of clients to determine care needs and develop individualized care plans.Train and supervise non-skilled caregivers in providing personal care, companionship, and assistance with daily living activities.Monitor clients' overall well-being and report any changes to the appropriate healthcare professionals.Educate clients and their families on effective care strategies, safety measures, and daily living skills.Ensure compliance with agency policies, state regulations, and best practices in non-skilled care.Maintain accurate documentation of client care and progress, while ensuring confidentiality.Act as a liaison between clients, families, and caregivers to facilitate communication and enhance the quality of care.Participate in ongoing training and professional development to stay current with best practices in home care.Ideal Candidate Will Possess:At least an BSN and current NYS License or compact License that is in good standing3+ years Nursing experienceAbility to travel within the 5 boroughsCurrent knowledge of state and federal home health regulations and community Qualifications:Strong interpersonal and communication skills.Ability to work collaboratively with caregivers and families.Proficient in electronic health record (EHR) systems and Microsoft Office Suite.Valid driver's license and reliable transportation required.Nice-to-Haves:Experience in geriatric care or working with individuals with disabilities.Previous experience in a non-skilled home care setting.Strong experience using a Uniform Assessment System (UAS);Proficient in Microsoft (Word, Excel, Outlook)Excellent verbal and written communication, organizational and time management skillsStrong critical thinking and problem-solving skillsBilingual in English and one of the following languages: Spanish, Russian, Chinese, Korean and BengaliExperience in Home Care, Geriatrics and Case Management; field experience assessing patient careWhy work at FreedomCare?Competitive salary and benefits package.Flexible scheduling to support work-life balance.Opportunities for professional growth and advancementWe are here for you and committed to providing a best-in-class employee experience. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $100,000 and $110,000 per year at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensurecertifications.Salary Range$100,000-$110,000 USD
