Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job DescriptionUnder the Senior Assistant Vice President of Managed Care Administration, the Director of VBP Policy and Operations will oversee the department's projects and strategies related to special populations, care management, policy, and financial operations.The Director of VBP Policy and Operations will be responsible for the following:Direct and manage the Value-Based Payment for Patients Experiencing Homelessness (VBP PEH) strategic work plans to promote overall total cost of care reductions and improved quality outcomes of homeless patients at H+H. In partnership with clinical co-champions, this includes design, oversight, and implementation support of strategies such as those related to insurance coverage optimizations, emergency department care management, high utilizer work group management, long term care placements, and durable medical equipment policy improvements.Provide coordination and support for the VBP PEH system-wide business model including maintaining strong relationships, collaborations, and work priorities with financial planning and operations, patient access, data and analytics, clinical and care models, and external and regulatory affairsManage internal and external VBP PEH relationships to oversee the program. Manage external relations with the MCOs, DHS, CBOsproviders, vendors, advocacy organizations, and consultants to advance the work. This includes managing the strategy and work plans of multi-stakeholder workgroups on financial performance, enrollment, care management, and population health management.Manage a direct report in developing, maintaining, and strengthening the Office of Managed Care Administration's work to support systemwide care management strategies and monitoring and tracking of value-based payment revenue targets and operations. A systemwide strategy on Care Management and Transitions, including supporting clinical leadership in steering committee management, advising on strategic focus areas for the work group, identify gaps and develop cross-department collaborations to close gaps, and promoting data-driven approaches to performance improvement and oversight across care management programs. Develop improved methods, tracking, and oversight of value-based payment revenue targets in the budget, financial plan, and business plans. Develop work plans and support implementation of projects that drive improved revenue performance of value-based payment risk pools including PCP alignment, HARP enrollment, SNP enrollment, and PCMH tracking. Support the department's policy work, developing processes for regular monitoring, advising, and consulting on managed care policy and policy opportunities that impact NYC H+H.Other duties as may be required. Minimum Qualifications1.
