The successful candidate will: Participate in quality and ambulatory care meetings.Provide Patient Outreach and appointment making for preventative and chronic care services, pre-visit reminders, follow-up after no-shows (via phone, letter, secure health messages andor patient portal). This includes collaboration with Downstate Managed Care office to improve NY QUARRHEDISPress Ganey scores and use of patient registries, online managed care portals, or other data tools.Provide Tracking and follow-ups on lab, imaging and specialty referrals; furnish information to specialties and obtain return of consults as needed.Assist clinicians with pre-visit planning, including checking patient information on the Regional Health Information Exchange, patient portal or other electronic systems, attending pre-session huddle.ProvideExchange of patient information with other care settings (in-patient, ED, post-acute care, specialties).Connect patients to needed clinical and non-clinical resources, such as social work, insurance assistance, medications, transportation.Participate in organizational trainings and PCMH initiatives.Other duties to be assigned by Manager in Ambulatory Care Leadership. Required Qualifications: High School Diploma or GED.Must be proficient with MS Excel and electronic records system and databases.
