This position is included in a bargaining unit of Housing Works' New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union ("RWDSU"). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a "CBA") between Housing Works and the RWDSU Compensation Range: $21hr - $24hr commensurate with experience.Overview: The Care Manager will provide all aspects of care coordinationfor SAMHSA AIM 2.0 clients: complete initial enrollment (all required SAMHSA and program formsassessments), develop care plans, promote compliance with appointments and treatment regimens, and encourage self-sufficiency.The Care Manager will assist clients to access services, such as primary care, substance use treatment, mental health counseling, transportation, intensive care management, housing and vocationaleducational resources, through phone calls to clients and providers, and home visits when needed. The Care Manager will also participate in outreach events as needed.Responsibilities: Provide supportive case management to SAMHSA AIM 2.0 clients, with a focus on providing access to and support for the SMIco-occurring disorder population.Provide care coordination on an ongoing basis with other service providers, participate in medical case conferences to promote integratedcare.Follow up with the client and the referral provider to ensure that appointments arekeptand that the patient is re-engaged in care and referred toneeded treatmentprevention.Conduct HIV and Hep C testing as neededPerform various administrative duties, including updating the client list and outreach logs; assisting with outreach calls, and scheduling intakes and follow ups.Maintain essential data for reporting and program evaluationin eICare, including program enrollment, progress notes,care plans,referral activity, and updating contact plete UND enrollments and PrEP referrals as determined by client needPromote sexual health and wellness through the use of risk neutral approaches and evidence-based interventionsBuild and maintain resource lists for external services and emergency services (i.e., housing, domestic violence, food pantry)Participate inscheduled supervisory sessions with the Program Coordinator, Clinical Supervisor, and other Departmental Leaders as neededParticipation in youth community events, such askikiballs, LGBT events, testing events, etc.Attend IDT meetings and daily huddles and complete updated info by due date in preparation for meeting.Facilitate groups and EBIs and complete documentation within 48 hours.Participate inreoccurringinteragency meetings, as well as external community meetings to improve integrated care andmaintain relationships with community partnersParticipate in department activities such as new staff onboarding, YPS Pride Committee, CQI efforts,etcParticipate in Housing Works advocacy events (minimum 3 in person per year, 4 via other mode July-June)Perform other duties as assigned Minimum Requirements Bachelor's Degreerequired, plus2 years of casemanagement experience preferred
