Details Client NameSchulman & Schachne Institute for Nursing and Rehabilitation Job TypeTravel OfferingNursing ProfessionLPN LVN SpecialtyLTC Job ID30971606 Job TitleSSI - LPN LTC Weekly Pay$1513.09 Shift Details ShiftMid - 8x5 - 02PM Scheduled Hours40 Job Order Details Start Date04082025 End Date10112025 Duration27 Week(s) Job Description Job Title: Licensed Practical Nurse (LPN) - Long Term Care Job Specialty: LPN in Long Term Care Job Duration: 27 Weeks Shift: 3pm-11pm, including every other weekend Guaranteed Hours: 40 hours per week Experience: Preferred 1+ years of relevant experience; nursing home experience required License: Licensed and currently registered as a Practical Nurse Certifications: BLS (American Heart Association) or NY State CPR certification Must-Have: Demonstrate competence in nursing skills within scope of practice, establish positive team morale and good interpersonal relationships with residents, families, and coworkers Job Description: - Perform tasks and responsibilities in case finding, provide supportive and restorative care under the direction of a Registered Professional Nurse.- Conduct nursing skills and provide nursing care to assigned residents per facility policy.- Enhance the quality of life of residents.- Function as a member of the nursing and interdisciplinary teams.- Provide direct nursing care to assigned residents with complex needs related to hygiene, comfort, medical treatment, nutrition, ambulation, exercise, elimination, and safety.- Direct the care of residents performed by Certified Nursing Assistants.- Administer medication, excluding certain specific treatments, and manage IV therapy as per certification.- Prepare residents for diagnostic tests, examinations, and transportation, and assist with admissions.- Participate in Person Centered Care planning, coordinate nursing care in assigned units, and implement plans.- Assist in health teaching for residents and families, and contribute to developing care plans.- Establish positive relationships with residents and families in accordance with residents' rights.- Participate in unitteam ICP meetings and improvement teams.- Monitor nutritional needs, assist at mealtime, and supervise staff during resident feeding times.- Assist in post-mortem care of deceased residents as needed.
