Job DetailsJob LocationLong Island City - Long Island City, NYPosition TypeFull-Time Non-ExemptSalary Range$45,000.00 - $55,000.00 SalaryyearJob ShiftDayDescriptionTitle: APIP Case ManagerUnit: Family ServicesReports to: Director of ATI and Family Services Status: Full Time; Regular; Non-ExemptSalary Range: $24.73 to $30.22 (approximately $45,000- $55,000 annually)Location: Long Island CityDaysHours: Varies:Monday through Friday, 9:00am - 5:00pm, 11:30am - 7:30 pm. Subject to change depending on program needs Position Summary:Responsible for facilitating Abusive Partner Intervention groups, case management duties including individual sessions, developing treatment plans, documenting client progress for reporting to the courts, and making referrals to internal and external services.Essential Duties and Responsibilities:Facilitate Abusive Partner Intervention groups using evidence based curricula; Perform general case management duties, including weekly individual sessions with clients, referrals to community based services, developing client schedules, and client centered treatment plans;Assist clients in identifying and prioritizing needs;Provide referrals to ensure all participants' concrete needs are addressed;Ensure accurate and timely data entry of all client information into the agency's database;Provide accurate and timely data and information to be included in court reports; Monitor participants attendance and compliance with mandate requirements;Conduct toxicology for clients as needed;Participate in case conferences for optimum collaboration among departmentsprograms and external agencies;Attend and participate in meetings, conferences, and trainings;Conduct weekly chart audits; Engage in hisher own ongoing professional development; andAdditional responsibilities as required. QualificationsQualifications:High school diploma or GEDHSE required, some college preferred;Experience and commitment to working with mandated participants with criminal justice involvement;Strong Microsoft Office and database entry skills;Strong assessment and engagement skills;Strong written and verbal communication skills;Bilingual EnglishSpanish a plus;Strong organizational and leadership skillsWe seek talented, dedicated individuals from all walks of life who possess a strong commitment to this mission.
