We are currently seeking a Full-Time RN Discharge Planner to join our team at Jessup Correctional Institution located in Jessup, Maryland. SCHEDULE8:00am to 4:30pm Monday through Friday May possibly move to 7:00am to 3:30pm schedule; depending on locationThe RN Discharge Planner is responsible for coordinating patient discharge planning from a correctional setting. The Discharge Planner will work under the direction of the Director of Nursing in collaboration with a multidisciplinary team to develop and implement a safe and appropriate discharge plan to promote continuity of care after release.
