We believe the hours for this role will be 8:00-430. This role will be working with one of the most diverse patient populations in our M Health Fairview system. The providers with OB-GYN Riverside have an outstanding relationship with all staff, and we are passionate about education and supporting the team as a whole. You will also be asked to travel to our Highland Park and New Brighton sites on occasionThis Visit Facilitator is responsible for supporting in person clinic visits; including communicating with patients prior to their visit and helping guide patients through the care experience. Visit facilitators capture pertinent medical information from the patient & details the information into the medical record. This role also helps ensure the exam rooms are stocked, equipment & supplies are available, assists with tasks that support the effectiveness & efficiency of the team. The Visit Facilitator may also assist in scheduling & coordinating appointments, preparing schedules and other delegated duties as assigned.Prepare patients for their appointments including: communicates with patients in advance of their scheduled appointment to ensure medical records are collected, received & available for visitAssist in rooming process by collecting & detailing pertinent information: reason for visit, medication review, vitals collection with automated vitals machine, & health questionnairesSupports the patient connection with the provider for the visit & acting as a resource while ensuring a positive patientprovider experienceCompletes visit wrap-upafter-visit summary is provided & patients are aware of recommendations for follow up. This may include assisting with scheduling & coordinating follow-up municates efficiently with patients, providers & care team, demonstrating emotional intelligence in person & with virtual technologiesRequiredAt least 1-year of customer service experienceProficient with technology (computer hardware and programs, smart devices such as phones or tablets)PreferredCurrent nursing, medical, social work, psychology, or pharmacy studentSuccessful completion of: NA, EMT, or HUC programExperience with Medical terminology, Epic Health Records, Telephonic customer experience and has previously worked in a health-care setting QualificationsCompensation$20.01 - $28.25Hr.
