The LPNRN coordinates physical, social, and emotional aspects of assisted living care and services in accordance with the physician's orders and the resident's choices.Responsibilities include but are not limited to:Execution of the medication management plan including periodic review of the documentation, observation of the effectiveness of the plan, communication to the physician when changes are needed or outcomes are not appropriateContinuous assessment of resident needs with updates to appropriate individuals as it relates to care, services and changes in conditionImplement measures to ensure resident wellness and safety and the appropriate documentation of those measuresWork in a team fashion to meet resident needsRespond to emergencies timely and in a calm and competent mannerReport incidents and changes in resident condition to Healthcare Coordinator and with the appropriate documentationBe an active part of all activities including community eventsBe present and assist with the delivery of meals in the dining room, monitoring resident acceptance of diets and assuring adequate nutritionMaintain high standards with regard to record keeping, resident and staff documentationEstablish and maintain a positive working relationship with all departmentsIdentify risk factors for residents and initiate interventions including the appropriate documentation of that interventionProvide a thorough report of resident status to on-coming nursing team, at each change of shift, and in the Daily LogProvide motivation and direction to CNARMA'sMust be certified or willing to take and pass the certification classes required by the state in which employedAttend required trainings, CEU's and staff meetingsAdhere to and implement all policies and proceduresPractice proper body mechanics and safe resident transferring techniquesAdhere to attendance policies; report to work on time and prepared to begin dutiesComplete all other assigned dutiesRequirementRequirements:Current RN, LPN licensed to practice nursing in the state in which the community is located, holding a license in good standing with the Board of NursingExperience working with the senior population, preferably in assisted living or LTCMust be a team player who works well with others and has experience supervising staffMust possess critical thinking skills, ability to effectively communicate verbally and in writingDemonstrated ability to manage time efficiently with strong organizational skillsMust be in good physical condition and be able to sit, stand, bend, kneel, walk, and lift up to 50 lbsMust be willing to work an 8-16 hour flexible schedule to include weekends and holidaysMust attend all required trainings, CEU's and staff meetingsMust be certified or willing to take and pass the certification class required by the state in which employed
