We are an employer of choice for anyone interested in a meaningful career in the social services field.QualificationsBachelor's degree preferredTwo years of related case management experience preferredProficiency in modern business communications including personal computers, electronicmail, voicemail, facsimile and copier equipmentBi-lingual Spanish-speaking strongly preferredEssential Duties and ResponsibilitiesProvide direct services to clients, including intake, engagement, annual assessments, serviceplan development, referrals, advocacy, exit summaries and follow-upManage a caseload of between 40-50 clientsMonitor client progress toward goals, document interactions with or on behalf of clientsaccording to program requirementsConduct home visits to clients as neededAccompany clients to appointments and act as an advocate on behalf of their health needs as neededMaintain familiarity with agency and city resources available to clientsAssist client with health care coordination, access to benefits, access to transportation, andother community resources as neededParticipate in regular supervision with supervisorPerform additional duties as assignedSkillsAbilitiesStrong writing and oral communication skillsAbility to use a computer for prolonged periodsAbility to occasionally lift andor move up to 10 poundsAbility to stand, walk, or sit for long periods of timeAbility to climb five flights of stairs, if required to conduct home visits andor fieldworkAbility to bend and retrieve objects andor documentsAbility to travel in the boroughs of New York City via public transportationAbility to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits andor field work BronxWorks seeks to build an inclusive organization grounded in respect for differences.
