Position Case Manager Description We are seeking a compassionate and dedicated Case Manager to provide comprehensive services to adults with severe and persistent mental illness (SPMI). This position is an integral part of an inter-disciplinary team focused on promoting recovery, wellness, and goal attainment. The Case Manager will work directly with clients to provide community-based supports, including resource development, crisis intervention, treatment planning, and promoting good health.Key Responsibilities:Provide case management services to support clients in managing severe and persistent mental illness.Develop and connect clients to resources that aid in their recovery, such as housing, healthcare, and employment.Promote and encourage healthy lifestyle choices, focusing on both physical and mental health.Offer crisis intervention in emergencies to de-escalate situations and ensure client safety.Work collaboratively with the interdisciplinary team to create and implement individualized treatment plans to meet specific client goals.Monitor client progress and adjust treatment plans as necessary to ensure successful goal achievement.Position Requirements Minimum of AABA required; LSWLPC, CCDCII or II strongly preferred. Experience with this population preferred.Experience working with adults with severe and persistent mental illness preferred.Strong interpersonal and communication skills.Ability to work effectively as part of an interdisciplinary team.Crisis intervention and de-escalation skills.Knowledge of community resources and healthcare services.Bachelor's degree in social work, psychology, or related field (or equivalent experience).Valid driver's license (if applicable).As a healthcare provider, Southeast Healthcare is a non-smoking employer.
