Supervisor-Case Management HFH

Duties: The Supervisor-Case Management HFH is responsible for overseeing daily caseSkills: as outlined in the Henry Ford Health Customer Service Policy and summarized below:Education: •Registered Nurse (RN) or a Licensed Social Worker (LMSW) with a valid, Schedule Notes: 6 openings for the same schedule at the same location; local candidates only, no travelers at this time This is only a 6 month contract, no opportunity for conversion MUST HAVE Epic and Careport experience MUST HAVE current acute care case management leadership experience (2 year minimum required) MUST HAVE inpatient hospital case management experience MUST HAVE discharge planning and utilization management experience MUST HAVE case management experience, no time to train on the basics MUST HAVE RN or LMSW license Master's degree required for LMSW; BSN required for RN Updated notes: Team is changing case management model from integrated case management to dyad model with SWs and RNs functioning at top of their license Since May, the team has onboarded 21 new employees and are looking for candidates to work at the elbow to mentor these new hires They will be a personnel manager, not a case manager, but need to have prior case management experience to counsel new hires on how to have difficult conversations with MDs and families dealing with complex discharges Must have recent acute care case management experience working full time in a hospital setting Must have leadership experience (in lead or manager capacity) Candidates that do not have to relocate to metro Detroit only at this time Candidates that do not have hospital experience following the height of Covid will not be a good fit Inpatient psych experience will not be a good fit as it is not the same as inpatient case management Hospital size of previous experience does not matterHours Per Day: 8.00Hours Per Week: 40.00Department: 201245 : Administration and Support HF Hospital _ Support ServicesJob Category: HFH-Clinical-Allied
