It focuses on promoting community integration, enhancing self-efficacy, and building a robust network of formal and informal supports. CTI teams conduct assertive outreach and engage patients with multiple behavioral health, medical, and social care needs, such as homelessness or justice involvement, with a particular focus on those who have had frequent contact with the healthcare system. The team supports patients during their transition from the hospital to the community, working to connect them with long-term, stabilizing resources. Ideal candidates for this role will be able to provide flexible patient-centered support in both clinical settings and in the field as needed.The Team Leader will provide oversight of services delivered by the team and demonstrate proficiency in ensuring that the team is delivering the CTI model effectively with fidelity. This role demands a dynamic and forward-thinking professional with a strong commitment to delivering high quality services to the systems most vulnerable patients. The ideal candidate will possess exceptional leadership skills, a strategic mindset, and the ability to drive impactful change across the services spectrum. This position requires a blend of clinical expertise, leadership skills, and administrative acumen to ensure the efficient functioning and the provision of high-quality careSUMMARY OF DUTIES AND RESPONSIBILITIES:Core Leadership ResponsibilitiesProvide clinical and administrative supervision for all CTI team members.Ensure team adherence to the CTI model, including fidelity to phase transitions and management of caseloads.Lead daily teamorganizational meetings and weekly service planning sessions.Oversee and manage team performance, including productivity, wellness, and timeleave responsibilities.Program Oversight & ComplianceMonitor and ensure completion of model-specific case planning and documentation by CTI staff.Coordinate team performance with the Office of Behavioral Health CTI Director.Ensure compliance with Office of Mental Health regulations and other applicable standards.Oversee performance improvement projects, utilization reviews, and incident management activities.Collaboration Build and maintain partnerships with healthcare providers, community organizations, government agencies, and advocacy groups to address care challenges, social determinants of health, and health equity.Work with facility-based leadership to implement special initiatives aligned with organizational objectives.Promote the program and raise awareness about available services.Advocate for policy changes and additional resources to improve services for the target population.Program Development & ReportingDevelop and implement plans to address crises or unforeseen challenges affecting the program or clients.Prepare reports for funders, stakeholders, and regulatory bodies to showcase program effectiveness and plete internal fiscal reports and manage Patient Service Dollars in collaboration with the Finance Team.Direct Support & Operational DutiesProvide coverage for patient caseloads as needed.Allocate time effectively between clinical and administrative responsibilities.Uphold high ethical standards and ensure program activities are conducted with integrity and respect.All of the above duties may change at the discretion of the Department Head or Supervisor.Minimum Qualifications1.
