They are seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) who is bilingual in Spanish to join their team. Job Responsibilities: Patient Care: Conducts comprehensive mental health assessments to diagnose patients and develop individualized treatment plans. Provides individual and group counseling and psychotherapy. Delivers crisis intervention services to address conflicts and urgent patient needs. Completes and maintains medical documentation, including referrals and transfer forms. Manages and tracks patient referrals to external mental health providers and agencies. Collaborates with specialists for consultations and referrals. Works with intake teams to determine the best course of treatment for patients. Participates in case reviews with clinical staff. Maintains confidential patient case files and ensures compliance with confidentiality policies. Adheres to clinical chart review policies and procedures. Works effectively in a team-based healthcare environment. Conducts workshops for families on communication and family-related topics. Coordinates emergency response efforts to ensure patient safety, including liaising with EMSFDNY, psychiatric or medical emergency departments, and follow-up care. Oversees the patient's mental health care and treatment plan within the scope of expertise. Promotes health education, disease prevention, and overall well-being. Provides patient advocacy as needed. Participates in case conferences and case management activities. Administrative Duties: Collaborates with Registration and Scheduling teams to ensure smooth patient scheduling and clinic flow. Works closely with the Medical Records Department to ensure accurate documentation. Ensures compliance with HIPAA and internal policies and procedures. Qualifications: Licensure: Unrestricted New York State Clinical Social Worker (LCSW) license. Experience: Minimum of two years of clinical experience preferred. Technical Skills: Proficiency in electronic medical records (EMR) systems and basic computer applications. Language Skills: Bilingual in Spanish required; additional language skills a plus.
