Founded in 1971, we believe that the quality of your health care should not depend on how much money you make, what language you speak or what your health is, because everyone deserves great care.Position Summary:Responsible for providing primary care in a family-based clinic environment in a holistic manner including the assessment of physical, psychosocial, health education, and discharge needs of for the patients we serve while maintaining a high level of professionalism and quality care across the life span in a clinic, virtual or communityshelter pensation is dependent on skills and experience.Your contribution to the team includes: Use client data and nursing scientific principles to develop nursing diagnosis and to identify client problems in order to deliver effective nursing care.Implement the plan of care by initiating nursing interventions through giving direct care and supervising other members of the care team.Collaborates with patient and Care Team for wrap around services.Performs independent visits while operating under standing order proceduresAssess walk-in patients for acuteness of need andor priority of intervention and document assessment information of all activities.Education of patients, families, and communities in a wide variety of health care subjects such as maternalchild health care, nutrition, hypertension, diabetes and lifestyle, sexually transmitted infections, basic health care maintenance, and self-administration of medications with chronic and acute conditions.Perform complex procedures as appropriate for the RN scope of licensure and the individual RN's competency (may include wound care, IV insertion and fluidmedication administration, urinary catheter insertion and removal).Perform patient follow-up including notifying patients of laboratory results, scheduling referral appointments, maintaining charts and records and responding to patient telephone callsMonitoring patients' ongoing conditions, adjusting treatment plans, and health promotion as neededProvide motivational interviewing and harm reduction counselingEngage, participate in improving the performance of identified quality measures and population health initiatives.Engage and collaborate in case management related to specific patient populations to ensure utilization of specialized healthcare services.Secondary duties:Triage of phone callsProvide OBGYN patient centered care.Attend staff meetings, in-service meetings and participate in agency committees and task force activities as required.Must have's you'll need to be successful:AAST degree in Nursing from an accredited college; or equivalent combination of education and experience. BSN preferredUnencumbered Washington State Registered Nurse license requiredCurrent BLS certificationProof of vaccination for COVID-19 is required, prior to start. Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to bloodblood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations andor immune titer results prior to starting or no later than their fifth (5) business day of employment.Where to gather your records: If you are providing immunizations from an electronic health record, please ensure that you obtain a copy of your full records rather than a screenshot. Each page of your records should include your first and last name, date of birth, and the name of the health system from which the records are pulled.If records do not show any data, please seek guidance from your provider for further assistance.If you are unable to provide proof as noted above, you can choose to have a lab titer drawn to check immunity or to be re-vaccinated.
