Life Enrichment Coordinator Shift: Days 9:00 -5:30 pm days off flexible, 16-24 hours a weekHourly Rate: $26. - $28San Francisco Campus for Jewish Living:Nestled in San Francisco's Excelsior neighborhood on a nine-acre campus, the San Francisco Campus for Jewish Living (SFCJL) is a center of excellence in providing a continuum of care. This includes the Frank Residences, offering luxurious assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specialized in mental healthcare for older adults over age 55; and the Jewish Home and Senior Living Foundation.Grounded in the Jewish values of dignity, compassion, and community, the San Francisco Campus for Jewish Living, fondly referred to in the past as "the Jewish Home," was founded in 1871. Today, 150 years later, it continues to build upon its legacy of enriching the lives of older adults.Essential Functions: Provide an opportunity for socializing and encouraging relationships among residentspatients. Create a positive, caring and stimulating environment. Re-motivate residentspatients to become actively involved in life, through contributing toward a renewal of physical, mental and social abilities. Respond to the needs and abilities of the residentspatients, which may vary and must be able to carry out activities with these differences in mind. Provide one-to-one relationship with the patientsresidents who decline to participate in group activities, or who are unable to attend group activities. Fulfill important emotional needs, towards residentspatients, by providing support in times of crisis and help with problems in daily living. Responsible for timely completion of resident'spatient's records to include Life Enrichment Initial Assessment, MDS, Progress Notes, Change of Condition, and attendance record and other activity notes as deemed appropriate. Responsible for creating, updating, and maintaining residentpatient centered Care Plans as scheduled. Work closely with other departments in the Home, in particular Nursing, Social Services, Dietary, Rehab and Volunteers, discharge planners, case managers, etc. to ensure the well-being and safety of residents and patients. Responsible for creating and maintaining activity boards daily with up-to-date information. Responsible for the distribution of residentpatient mail, weekly menus, activity calendars, and refreshments. May supervise student interns and volunteers who are involved with activities. Consultation with the resident'spatient's family members. Perform other duties as assigned.Qualifications: Bachelor's degree in area related to field (recreational therapy, psychology, health care, geriatrics, social work, or recreation administration), preferred. Two years' experience working within a geriatric group and related field. Sound group work, organizational and communication skills. Creative, flexible, energetic, enthusiastic, resourceful, self-motivated, innovative thinker, with a demonstrated ability to work effectively, independently and within an interdisciplinary team work approach. Must be sensitive to varying levels of physical and emotional health.
