The Medical Coordinator is a key member of the Supported Living team, this role's main functions are to coordinate and oversee the medical needs of the people we support. The Medical Coordinator provides guidance to the Direct Support Professionals and helps them to understand the directives given by medical professionals. The Medical Coordinator will be working at various sites in Manhattan, mainly in Washington Heights and East Harlem, Monday through Friday from 9 am to 5 pm, although there may be times when the needs of the program may require the person in this role to work a flexible schedule. Responsibilities include (but are not limited to) the following tasks:Medical AppointmentsAct as the liaison between clinical and residential services regarding medical appointments, advocating for the individual, meeting with the RN on a weekly basis to review medical appointments and discuss concerns of each individual.Schedule medical, dental, podiatry, psychiatric, and any other clinical appointments. Ensure all follow-up visits are done in a timely manner and that all the results of visits, lab work, etc. are appropriately documented and filed.Ensure that all appointments are recorded on the calendar and consult sheets, and that the clinic summary visit form is completed properly including doctor's recommendations.Ensure that all doctor's recommendations are followed up with after an appointment. Seek clarification and document when needed on the consult sheet. Accompany individuals to medical appointments when needed.Medication OversightAdminister prescribed medications and treatments; assist individuals in achieving more independence in administration of medications; teach uses of skin care lotions, topical ointments, eye and ear drops and assist in their use as needed.Ensure all prescribed medication stock is identified, maintained and stored properly in consultation with the nurse.Keep track of medication inventory and reorder medications as needed to ensure there is enough supply available on site.Train and supervise AMAP staff to monitor medication inventory while on shift. Discard medication, including controlled substances, in accordance with the nurse's instructions.Obtain updated prescriptions as munication and CollaborationCommunicate the implementation of new medications andor treatments and any potential side effects to all individuals and staff; teach, observe, and report problems and progress in self-administration of medications programs;Communicate any new diagnosis to staff and provide relevant information that may contribute to offering individuals the least restrictive level of support.Coordinate with the nurse and the supervisor the need for specialized training related to the individual's medical needs based on new diagnosis, observation, or feedback from municate with the individual's family about medical related concerns.Collaborate with the interdisciplinary team involved in the individual's circle of support, including the nurse, the behavioral specialist, medical providers, mental health professionals, nutritionist, physical therapists, occupational therapists, creative arts therapists, among others to ensure a holistic approach to the person's medical care.Provide training to Direct Support Professionals in Choking Part II and in how to effectively support individuals at medical appointmentsAdministrative TasksComplete and track all annual psychotropic, sedation (MIPS), and medical consent forms.Ensure all first aid personal protective equipment (PPE) supplies are available at all times, and that the inventory is maintained at a suitable level.Maintain accurate reports of weight, fluid intake, menses chart, behavior charts, and other significant symptomshealth-related occurrences.Observe and report on behavior and specific responses to programming, medication, interpersonal relationships, etc.Reports unusual incidents and physical or behavioral symptoms promptly and accurately to the nurse and supervisor.Maintain confidentiality of information.Actively participate at staff meetings by communicating observations concerning progress and reaction to various therapy programs.Qualifications:High school or equivalent is requiredBachelor's Degree (or equivalent experience) and at least one year of experience of working with individuals with Autism and Developmental DisabilitiesBackground in healthcare coordination, patient advocacy, andor medical compliance is preferredDemonstrated competency maintaining document records in accordance with program and OPWDD mandatesStrong written and verbal communication skills, along with strong follow through abilityWorking knowledge of Google Workspace a plus, especially proficiency in Gmail, Drive, Sheets, and CalendarMust be able to obtain and maintain AMAP, SCIP, CPR and First Aid certifications, trainings will be provided by the agencySalary: $47,800.00 annual salary, non-exemptBenefits: Health, Dental, and Vision insuranceShort Term Disability, Long Term Disability, Life and AD&D PolicyWorkLife Assistance Program (EAP)Pre-tax commuter and medical plans403b retirement plan with 5% employer match after two years of employmentGenerous vacation policyTo Apply: Job Path seriously reviews all application materials with an emphasis on the content the screening questions.
