Job Description Contacts and schedules patientclient's appointments for consultations, meetings with community agencies andor diagnostic tests. Sets-up and maintains files and records. Collects and organizes statistical reports. Job Responsibility Contacts patients; schedules appointments for consultations, agency meetings andor diagnostic tests. Collects and organizes data required for statistical reports and completes reports, as required. Attends and participates in meetings as appropriate. Sets-up files and maintains records. Provides incoming and in-process orientation. Based on position, may be required to provide bi-lingual translation andor guidance. Performs related duties as required. Job Qualification High School Diploma or equivalent required. 0-1 years of relevant experience, required. Ability to travel to Queens, Nassau and Suffolk as needed. NYS Driver's License, highly preferred. Previous experience in HIV case managementprevention, preferred.
