The Health and Wellness Cluster is a $10M division which provides a wide array of personalized behavioral health services to individuals coping with the challenges brought on by emotional problems and mental illness. A New York State certified Article 31 mental health facility, this division delivers outpatient mental health services, PROS, crisis services, HIV family mental health services, vocational rehabilitation, parent education, supportive employment, case management, screenings for entitlement benefits, health care navigator program and Senior Services. Services are provided by culturally competent staff who can communicate to our clients who need to communicate in Chinese and Spanish as well as English. These programs are designed to help individuals attain their fullest potential in an environment that values respect, dignity, caring and integrity.Job Summary:You will be working in the exciting new DOHMH program called CONNECTClinic -without-Walls which launched in January 2022. This project aims to provide rapid access to services and a more flexible and holistic approach to mental health that is more engaged with the community and the whole person outside the clinic walls and beyond the traditional doctortherapist-patient interaction. The purpose of CONNECT is to improve the quality of life of the people served as demonstrated by progress towards recovery goals and informed by process metrics related to improved access, engagement and retention in clinical and non-clinical services and reduced mental health emergencies, inpatient and mobile crisis utilization.As a Clinician in our clinic, you will provide clinical services to individual, groups and families. You will be asked to work in an interdisciplinary team to assist individuals and families to achieve their personal highest potential and live fully fulfilling lives. Intake work and documentation compliance are required tasks. As a Clinician you will be responsible for counseling and providing treatment to individuals with substance use, including use of tobacco, alcohol and other drugs. You will help patients with their addiction by educating them with the skills or the mechanics to address their Substance use issues and facilitate group sessions. You will assist Criminal -justice involved individuals accessing and obtaining mental health services and connect them to other resources (social determinants of health) We are a community mental health clinic with the mission to be accessible to the Lower East Side community.Qualifications:LMSW, LMHC, or LCSW required.Experience working in a substance abuse andor forensic setting.CASAC (Certified Alcohol and Substance Abuse Counselor) certification is a plus.Bilingual EnglishSpanish preferred to support diverse community needs.Familiarity with Electronic Health Records (EHR) systems is a plus.Experience utilizing Evidence-Based Treatment (EBT) modalities in behavioral health settings.Ability to work collaboratively as a team player within an interdisciplinary team environment.Strong interpersonal and communication skills to engage clients and work with diverse populations.Responsibilities:Individual & Group Therapy: Provide individual and group counseling services with a specific focus on the forensic population and individuals involved in the criminal justice tegrated Treatment: Deliver integrated treatment for clients with co-occurring disorders (mental health and substance use issues), ensuring a holistic and comprehensive approach to care.Service Development: Assist in the development and implementation of integrated treatment services within the Article 31 clinic to address the unique needs of the population served.Clinical Case Management: Provide clinical case management services as needed to support clients in achieving their treatment goals and connecting to appropriate resources.Documentation & Compliance: Maintain all clinical documentation and paperwork in accordance with State regulations, ensuring timely and accurate record-keeping.Collaborative Treatment Planning: Engage in collaborative documentation with clients during therapy sessions, encouraging their participation in the development and reviewing their treatment plans.Staff Meetings: Actively participate in mandatory staff meetings, contributing to program development, case discussions, and team coordination.Afternoon Shift: Work at least one afternoon shift (1pm-8pm) per week to accommodate client needs.Other Duties: Perform other related duties as assigned by your supervisor to support the clinic's overall operations and goals.Essential Physical Job Functions:Ability to walk on sidewalks, up and down stairs, and throughout the clinic consistently throughout the day. Ability to carry folders, a tablet, a notebook, and a cell phone (up to 20 pounds) for several hours per day.
