Performs major duties and functions as described in the Staff Nurse position description.2. Assists in coordinating, guiding and directing the development, implementation and continuous evaluation of the nursing regimen for assigned patientsclients and others within the unit.3. Conducts conferences to develop nursing care plans and to promote staff development.4. Performs other professional nursing activities as delegated by nursing service administration. SUMMARY OF DUTIES AND RESPONSIBILITIES:Under the supervision provides professional nursing care to patients within as assigned clinic. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case finding, health teaching, health counseling and provision of care supportive to and restorative of live and well -being. Serves as team leader and assist Head Nurse Nurse Manager in guiding and directing nursing personnel in nursing care activities. SPECIFIC DUTIES AND RESPONSIBILITIES:Practice of CaringHealing Model:Demonstrates authentic presence and attentive listening.Accurately identifies and addresses person by name.Performs major duties and functions as described in the Staff Nurse position description.Demonstrates Standards of Behavior using I CARE values with Kindness Critical Thinking:Ability to recognize changes in patient condition and take appropriate intervention.Ability to delegate tasks to appropriate staff and consistently demonstrates accountability for delegated responsibilities.Reports significant issues and follows appropriate communication channels.Verbalizes knowledge of the escalation policy.Care ManagementDisease Management:Appropriately communicates patient's condition and interventions done with care teampatientfamily and document in EMR (EPIC). Collaborates with patient family to develop individual care plan, including treatment goals reviewed and updated at each relevant visit.Assesses and addresses barriers when patient has not met treatment goals.Identifies patientsfamilies that might benefit from additional care management support.Supports patients and families in self-management, self-efficacy and behavior pletes staff assignments to provide adequate clinic coverage.Participates in direct patient care where demonstration of skills and current approaches to nursing techniques are required.Documents in a manner that meets medicallegal and regulatory agency requirements.Provides timely advice via telephone triage.Adheres to rules pertaining to the confidentiality of patient information and patient rights.Enforces hospital policies and procedures.Participates in providing safety measures for the safety and well being of patients, visitor and staff in Subspecialty Clinic. Patient and Family TeachingSelf Care Support Process:Uses TEACH BACK in evaluating patientfamily education outcome.Provides educational resources that are relevant and needed by the patient to achieve treatment goals.Safety Awareness:Demonstrates an understanding of hospitaldepartment policiesprocedures and regulatory standards.Demonstrates knowledge of the Joint Commission National Patient Safety Goals.Demonstrates knowledge and application of infection control standards.Leadership:Assumes charge responsibilities in the Subspecialty clinic. Promotes standards of Ambulatory Care Nursing as deemed clinically appropriate.Provides delegation of duties and direction to staff in all aspects of patient care.Assist in the review, revision of Policy and Procedure as it relates to Subspecialty Clinic Applies appropriate problem-solving methods in assessment and resolution of issues.Prepares and review daily staffing plan in Subspecialty Clinic.Monitor staff time and attendance in Subspecialty Clinic. Conduct regular unit council meeting and with written agenda. Participates, supports and integrates Breakthrough principles into clinical practice. Participates in the TeamSTEPPS Program to promote communication, safety and teamwork.Collaborates with multidisciplinary team including medical staff and leadership.Demonstrates ability to participate in quality improvement initiativeset goalstrack and assess results.Demonstrates ability to apply and role model standards of community behavior (I CARE) pletes employee's annual competency record and functional evaluation including department specific training record.Assure policies & procedures related to the Corporation's privacy and security practices.Assist in the review, revision of written Policy and Procedure as it relates to Subspecialty Clinic.Responsible in reviewing and monitoring messages from My Chart and Contact Center for Subspecialty ClinicProfessional Development:Evaluates own needs for learning and seeks appropriate resource(s) internal and external to maintain level of competency.Knowledgeable and compliant with Joint Commission Patient Safety Goals.Knowledgeable about ANA Standards of Nursing Practice.Maintains membership in a professional organization.Participates in community activities.Accepts responsibilities for insuring current license, BCLS certificate are on file and timely completion of annual mandatories such as QHC Annual Mandatory, Department Specific job training and OHS medical clearance.Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and3.
