We're looking forward to getting to know you!For more information, visit Description:Oak Street Health delivers high-quality primary care to Medicare patients in neighborhood centers - and at home when needed. The House Calls Medical AssistantCIS will fulfill a crucial role, supporting logistics, clinical care, and documentation in our innovative home-based care program.
