Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Work ShiftsTour II (9:00 A.M - 5:00 P.M) Job DescriptionSUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:Collaborating with community agencies (i.e., ACS, OMH, OPWDD, etc.) conducting initial psychiatric assessment for children and adults, safety plans, documenting biopsychosocial assessments, formulation and disposition, developing comprehensive treatment plans, facilitating supportive, psychoeducational and insight-oriented psychotherapeutic interventions, providing crisis interventions to high-risk patients, engaging with and providing support to families, and engaging them as partners in recovery.Identifying and collaborating with existing support systems, facilitating community referrals as needed, collaborating with interdisciplinary treatment team to develop most appropriate case disposition, documenting all interventions within the required regulatory timeframes. Participates in the evaluation of treatment progress. Accurately and timely records treatment planning, consistent with established standards, applicable administrative guidelines and regulatory requirements. Coordinates and provides case and care management services and activities; makes referrals and itiates and finalizes discharge and transition planning, in collaboration with patient and clinical treatment team.Maintains timely, accurate, and appropriate documentation in compliance with all applicable standards to ensure appropriate reimbursement.Provides orientation to new patients, patients' families and significant others; discusses financial responsibility with patients and provides money management assistance.Provides crisis intervention and support by responding to and following through in crisis situations; demonstrates good clinical judgement by seeking additional assistance andor consultation in the management of crisis situations, when needed.Participates in relevant staff meetings, case conferences and trainings; participates in special projects and internalexternal quality assuranceperformance improvement activities.Holds and maintains required certification(s) and completes required continuing education; submits appropriate documentation within established timeframe. Complies with NYC Health + Hospitals policies and procedures, and applicable to local, state and national requirements and guidelines.Performs other related duties, as assigned.Candidate may be assigned to work in other clinical areas as needed when patient care needs arise.
