JOB SUMMARYProvide emotional support to seniors 60 years and older in crisis or in at-risk situations. Provide services to stabilize the senior through referrals and resources. Assist seniors with identifying services to maintain independent living. ESSENTIAL DUTIES AND RESPONSIBILITIESMust do in-home assessments. Observes client home environment and interaction with family membersneighbors.Records observations of home condition, family behaviors and relationships in readable and accurate "Case Notes" form.Maintains contact with Chicago Department of Family and Support ServicesSenior Services as necessary to coordinate client services through various agencies.Works collaboratively with partners as well as developing new partnerships within The Salvation Army and the community to provide resources for participants.Maintain in a timely manner, required documentation of participant's involvement including but not limited to assessments, demographic data, case notes, closing summaries, and outcomes data collection. Seeks and identifies community resources and encourages client to utilize them to enhance their functioningActs as a liaison between client and various organizations to facilitate communication and understanding. Enters all information into the city data base in a timely fashion. Obtain appropriate releases of information. Conducts outreach activities to bring awareness and participation in the program. In consultation with the Senior and In-Home Services Program Manager initiates contact with the Illinois Department of Children and Family Services as a mandated reporter of suspected child abuse or consultation with the Senior & In-Home Services Program Manager, initiates contact with the Elder Abuse Hotline as a mandated reporter of suspected elder abuse.Participate in weekly supervision and case consultation with Senior & In-Home Services Program Manager. Seek consultation from the Program Manager as needs arise. Attends Senior & In-Home Services staff meetings as scheduled. Maintain awareness of applicable liability issues and other applicable legal issues, and potential effect on clients andor the agency.Abide by the ethical and culturally competent standards as outlined by the National Association of Social Workers and The Salvation Army.All other duties as assignedIntra-Agency ResponsibilitiesAwareness of agency policies and procedures.Attend agency general staff meetings and professional trainings as applicable.Perform special administrative assignments or duties as may be delegated.Collaborate with other Salvation Army units and personnel to successfully maintain and achieve policies and procedures, goals and objectives, and outcome-based measures related to City Mission philosophy.Understands, and can effectively work under, the Mission of The Salvation munity RelationsAttend meetings with community partners, as needed.Participate in community events as part of public relations and marketing of the Senior and In-Home Support Services Program.Develop partnerships and resources with other service providers.Other duties as assigned.RELATIONSHIPSThis position reports to the Senior & In-Home Services Program Manager. Receives referrals from the City of Chicago Department of Family Support Services (DFSS)Accepts referrals daily from city of Chicago DFSS CAS (Case Advocacy and Support) team and will consult with the team for support when in the field. Administrative supervision is provided by the Program Manager of Senior & In-Home Support Services based upon assigned duties. In contacts related to this position's duties this individual acts as a representative of The Salvation Army and its mission.PERFORMANCE MEASUREMENTSThis individual will be evaluated on how effectively the outcomes of this position are achieved, as well as the timeliness, accuracy and completeness of accomplishing assigned goals.EDUCATIONEXPERIENCEMinimum of a BA or BS with a MA, MS or MSW preferred. Experience with working with seniors and families.Driver's license, MVR clearance and a working, appropriately insured PENTENCIESKnowledge of the components of senior services and the resources available.The ability to effectively integrate program services with organizational concepts.Demonstrated ability to use initiative, work independently, and be a self-starter.An interpersonal style that promotes collaborative and effective team relationships.Flexibility in accepting assignments with various types of clients and environments.Must be sensitive to and work appropriately with culturally and socio-economically diverse client populationWarmth and a strong commitment to helping people, along with acceptance of varying lifestyles.Willingness to work with seniors in inner-city neighborhoods. Excellent communication (verbal and written) and interpersonal skills.An ability to use technology to conduct interviews and group meetings.An ability to work with confidential material.A willingness to support the mission of The Salvation Army.POSITION LIMITATIONSThis individual will only commit Army resources that have been allocated or approved.This individual will keep the Program Manager of Senior & In-Home Support Services informed on all critical issues relating to the area of responsibility.This individual will adhere to all policies and procedures in carrying out the responsibilities of this position.PHYSICAL DEMANDSWORK ENVIRONMENTThis position is required to do light physical work. In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and a PC.
