PurposeTo develop, coordinate and implement programs and services for residents with cognitive impairment; to provide a therapeutic environment that maintains each resident's highest level of well being of mind, body and spirit.Essential FunctionsJob FunctionsDirect and supervise Memory care staffCoordinates the implementation of the Memory Care Program mission, goals and philosophyProvides or secures training to associates in the Community including training caring for residents with memory impairment and other advanced trainingProvides or secures the additional trainingpracticum experience to Resident Caregiver associates of Memory Care ProgramCoordinates with Director of Resident Care in establishing and maintaining a staffing pattern to meet our customized care commitmentMaintains a mentally, physically and spiritually stimulating activity program that is geared to the individual's preferences and capabilitiesParticipates with the Director of Resident Care in the hiring, scheduling and supervising of Resident Associates assigned to the Memory Care programActs as liaison to facilitate communications among Memory Care, the community, family members and the local communityMaintains as environment in the residents' personal and congregate areas, which will help to compensate for cognitive, sensory and physical impairmentsInterviews residents and families prior to admission. Coordinates the pre-admission screening and assessment process with Directors of Marketing and Resident CareCoordinates with the Director of Resident Care, Memory Care Program Caregivers and families in the development of the 24-hour plan for the residents and updates the plan in a timely mannerCoordinates delivery of services for each resident according to the 24-hour planAttends Department head meetings and leads regularly scheduled Memory Care Program meetingsProvides Caregivers with updates and in-service training programs and Memory Care programsPursues educational development in Alzheimer's disease and related disordersAssists the Marketing Director in promoting the Memory Care Program to doctors, hospital discharge planners and local community agenciesParticipate as instructed in the sales process of potential residentsCoordinates the Memory Care Program volunteermentor programDevelops and maintains the community's resource libraryModels our company philosophy, helping out whenever needed to meet residents' needs and working as a member of the overall teamParticipate as a Manager on Duty during assigned weekendsAny other duties as assigned by the Executive Director andor Director of Health and WellnessQualificationsMust be 21 years of age or olderKnowledgeable of applicable state regulationsMust read, write, speak and understand EnglishBachelor's Degree in social Work, Psychology, Gerontology, Therapeutic Recreation or equivalent is desired. Previous successful management of a Memory care unit is requiredKnowledge and experience in working with older adults with memory impairment; at least two years experience in geriatric careAbility to communicate individually, and in group settingsMaintains professional conduct with associates. Supervisory experience desirableMust have upper body strength adequate to bend, lift, shift, move, andor assist in moving articles of more than twenty-five poundsMust be able to squat, reach, stretch without distress, Must be able to tolerate extended periods walking, standing
