Details Client NameSchulman & Schachne Institute for Nursing and Rehabilitation Job TypeTravel OfferingNursing ProfessionLPN LVN SpecialtyLTC Job ID30803202 Job TitleSSI - LPN LTC Weekly Pay$1504.15 Shift Details ShiftMid - 8x5 - 03PM Scheduled Hours40 Job Order Details Start Date03112025 End Date09092025 Duration26 Week(s) Job DescriptionJob Title: Licensed Practical Nurse (LPN) Long Term CareJob Specialty: Sub-Acute NursingJob Duration: 26 weeksShift: Days, 3pm-11pm (No exceptions at this time, must be able to work every other weekend)Guaranteed Hours: 40 hours per weekExperience: 1+ years relevant experience preferredLicense: Licensed and currently registered as a Practical Nurse Certifications: Basic Life Support (BLS) or current CPR certificationMust-Have:- LPN licensure - Certification in IV therapy- CPRBLS certificationJob Description:- Perform tasks and responsibilities in case finding, and provision of supportive and restorative care under the direction of a Registered Professional Nurse.- Deliver nursing care to an assigned group of residents in accordance with facility policies.- Enhance the quality of life for residents.- Function as a member of the nursing and interdisciplinary team.- Provide direct nursing care to assigned residents with complex needs related to hygiene, medical treatment, nutrition, ambulation, and safety.- Direct and oversee care provided by Certified Nursing Assistants.- Administer medication to assigned residents, excluding blood products and certain IV medications.- Administer IV therapy as certified.- Prepare residents for diagnostic tests, procedures, medical exams, and transportation.- Admit residents and assist them in adjusting to the environment.- Participate in person-centered care planning and implementation.- Assist and contribute to the development and updating of resident care plans.- Support RNs with health education for residents and families.- Establish positive relationships with residents and families.- Participate in unitteam meetings and performance improvement initiatives.- Observe and respond to residents' nutritional needs and actively participate during mealtimes.- Assist in post-mortem care as needed.Skills:- Demonstrates competence and experience in nursing skills within the scope of practice.- Ability to establish positive team morale and good interpersonal relationships with residents, families, and co-workers.
