We work every day for a fair and just society where all people have the opportunity to make choices that lead to better lives for themselves and their families.Program Description: The Assertive Community Treatment (ACT) Team is a unique and nontraditional licensed outpatient psychiatric clinic for individuals who have demonstrated a high service need with multiple hospitalizations, emergency room visits and difficulty maintaining stability in the community. ACT is a nationally recognized model and an evidence-based practice. The ACT Team serves the Upper West Side, Harlem, Washington Heights and Inwood.Purpose of Position: The Psychiatric Nurse Practitioner is responsible for delivering direct and comprehensive psychiatric care for the ACT team clients which includes prescribing medications and completing psychiatric evaluations.Schedule: 14 hoursweek. Flexible.Hourly Rate: $115.00 - $135.00Roles, Responsibilities, and Essential Duties1) Psychiatric Provide psychiatric treatment to 34 clientsConduct monthly psychiatric sessions with each clientPrescribe medication when appropriateComplete psychiatric evaluations as neededAssess psychiatric and medical status of clientsComplete required paperwork following program guidelinesCommunicate with clients' families about their needs and progressServe as liaison to other service providersVisit clients in residential facilities, on the street and in other locationsBe available by telephone after hours an on weekends to assist the team with urgent client interventions2.
