We want to help our patients get all of the health care they needwhen they need itso they may too live a healthier and happier munity Health Services, Inc. is seeking a Care Coordinator for our Pediatric and Adolescent Medicine Department. The incumbent's primary responsibility is to consistently provide patient-centered administrative support to the department in patient care coordination processes. Different tasks included in the Care Coordinator role are related to home care orders, prior authorizations, scheduling hospital discharge follow up appointments and developing and implementing Population Health Management Initiatives. Position reports directly to the Pediatric Nurse Manager. This multi-disciplinary work will be in alignment with Community Health Services' Organizational Core Values and the Patient Centered Medical Home approach to care.The successful candidate will:Coordinate referrals for medical services as necessary with external providers and assist patients with pre-visit planning, including prior authorizations, transportation needs, confirming appointments, obtaining medical records, and completing any other necessary paperwork or activities to eliminate barriers to careCompile and provide patient records to external providers as required complete referralsEnsure that referrals are addressed in a timely manner by submitting, processing, tracking, documenting, and following up on all referrals requested by providersCollaborate with medical records to ensure reports, test results, and other documentation is captured in the electronic health recordEnsure closed-loop communication through correspondence with patients and with internal and external providers regarding appointment time, location, directions, and the overall status of referralTimely document care coordination activity, including referrals, hospital admission, outcomes of services delivered and other information, into electronic health record as appropriateCommunicate with patients to identify and provide assistance in resolving potential barriers to care, including financial issues and issues with language andor transportationFacilitate a smooth transition into the organization from inpatient or emergency department visits by using various databases to obtain and review admissions and discharge summaries and working with reception staff to schedule appointmentsAssist patients in attaining access to care and services, by advocating for patients with insurers, providers, and governmental agenciesMaintain knowledge of health care services and providers in the community and the resources available to support patients' social determinants of health needsDevelop and maintain positive relationships with partners in the community who can provide healthcare and other services to patientsParticipate in outbound calling as assigned or as necessary to confirm appointments or bring patients into careWhen needed, act as backup resource for incoming telephone calls to the nurse, addressing, transferring, and otherwise managing calls as appropriateCommunicate with patients in their preferred language, using interpretive services as necessary, and interpreting for provider if proficiency in language has been establishedContribute to population health efforts by extracting and analyzing patient utilization information from the electronic health record, data analytics software, and other systemsPerform all duties in accordance with CHS Core Values, Compliance Program, and other policies and proceduresParticipate in quality and process improvement efforts in the department and the organization as requested or as directedComplete training and professional development requirements in a timely mannerHigh school diploma or equivalentCPR certification at the healthcare provider level or eligible to obtain certification within 90 days of hireProficiency in the use of Microsoft Office applications, including Word, Excel, and OutlookThe ideal candidate will also be a Medical AssistantSpanish speaking candidates preferredSome benefit programs available to eligible employees include:Medical InsuranceDental InsuranceVision Insurance401(k) Plan and up to 4% Employer MatchGenerous Paid Time Off9 Paid HolidaysCompetitive PayEmployer Paid Life InsuranceEmployer Paid Short Term DisabilityEmployer Paid Long Term DisabilityEmployee Assistance ProgramsWorker's CompensationVarious Leave Programs - Bereavement, Military, Maternity, and moreCOVID-19 Vaccination Requirements:CHS requires all new employees to be vaccinated against Covid-19 prior to starting employment with CHS unless they are approved for a reasonable accommodation based on disability, medical condition, or religious belief that prevents them from being munity Health Services is An Equal OpportunityAffirmative Action Employer.All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability or veteran status, age or any other federally protected class.
