We are seeking a Psychiatric Nurse Practitioner to provide mental health care at local shelters and drop-in centers serving homeless clients in the downtown Minneapolis area. In this role, you will primarily assess and provide ongoing psychiatric treatment for homeless persons with mental illness. For more information visit .Location and hours:This position is hybrid and will be performed both on-site at various Health Care for the Homeless (HCH) sites, throughout the community at shelters and drop-in centers to provide mental health services, and remote as job duties require. Work hours are primarily days, Monday through Friday, to be allocated as needed to fill in for staff, with some evening hours required. No weekend, overnight or holiday shifts are required. If possible, the candidate would be able to have at least one set workday a week, but this is flexible. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. About the position type:This is a limited duration (up to two years), non-benefit earning position.This position is internally classified as a Nurse Practitioner.To view the job classification, click In this position, you will: Perform initial and ongoing assessments of patients' psychiatric and psychosocial statusProvide ongoing medication management for patients including assessing effectiveness and adjusting medications for optimum effectiveness with minimal side effects; consult with other providers and team membersProvide direct care, monitoring, andor referral for co-occurring physicalmedical conditionsProvide consultation to medical providers on psychiatric patient concernsServe as an active team member of a multi-disciplinary team by attending meetings; actively participating in program planning, development and evaluation; and assisting with reports and special projectsNeed to have:One of the following:Possession of a valid license as a Registered Nurse issued by the State of Minnesota and National Certification as a Nurse Practitioner (CNP).Possession of a valid license as an Advanced Practice Registered Nurse issued by the State of Minnesota.National Certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP).Nice to have: CPR certification.Post-degree, full-time professional nursing experience within psychiatric mental health.Experience:Providing trauma-informed care.Working with people experiencing homelessness.Strong leadership, verbal, and written communication skills.Flexibility to adapt in a changing environment to meet patients' needs.Ability to work independently, counsel and instruct individuals in a state of stress, and act as an advocate.About the department:Hennepin County Public Health's vision is for all people who live, work and play in Hennepin County to experience optimal health grounded in health and racial equity.
