Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job DescriptionPOSITION OVERVIEW: The Non-Clinical Admissions Liaison is be responsible for managing admission referrals from primary referring hospitals, and ensuring a smooth discharge process for patients transitioning from a hospital setting to assigned PAC facilities.SPAN OF CONTROL: Will supervise one Admissions Coordinator (Assistant HCCPA)ESSENTIAL FUNCTIONS: •Ensures the completeness of a referral package in CarePort or eFax software upon acceptance of an admission and prior to transfer of hospital patient to Post-Acute facility. A complete referral package will include all received documents (Admission application, PRI, Screen, HP, face sheet, Covid swab if available, medication administration record, discharge medication list, discharge summary, EPACES if pertinent, Ability, insurance authorization, or denial letter (if applicable), CarePort communications) are upload in PCC.•Ensure that referral package in CarePort or eFax are sent out by admission liaison prior to transfer of hospital patient to Post acute facility.•Establishes the patient's insurance eligibility for Post-Acute care Communicate to hospital discharge team the type of authorization needed for Post-Acute admission.•Verifies health insurance coverage for patients prior to acceptance•Interacts with a variety of hospital social servicecase management staff regarding patients discharge plans•Screen referrals for suitability of prospective admissions.•Communicate Post-Acute care to patients and their families as needed.•Coordinates with healthcare facility staff to ensure that each patient has a smooth transition through the discharge process.•Coordinates communication between patients and their physicians, case managers and other medical staff.•Have proficiency in the use of CarePort to manage referrals•Maintain knowledge of clinical capabilities and services of NYC H+H Post-Acute facilities•Obtain accurate information from physicians, patients, and referral sources regarding the expected discharge plan, and communicate this information to Post-Acute facility as needed.•Develop relationships with hospital social workers, physicians, hospital discharge planners and administrators, clergy, attorneys, home health agencies, families, etc. to understand the need for program services and to communicate services offered which meet these needs. Participate in the growth of hospital referrals and admissions, cultivating relationships with sources to generate repeat referrals.•Communicates activities and concerns to supervisor as necessary•Completes required paperwork, manages correspondence and maintains data as required•Provide cross coverage for other Admissions department staff as required.Minimum QualificationsMINIMUM QUALIFICATIONS 1. Master's degree from an accredited college or university; or2. Baccalaureate degree from an accredited college or university; and one (1) year of administrative and operational support or related experience; or3. A satisfactory equivalent combination of training, education, andor experience, in which each year of relevant training andor experience can be substituted for 30 semester credits from an accredited college or university.
