School Trach and Vent 1.1 LVN

Comfortable and experienced with Vent and Trach 1:1 cases Cases available in Tustin, Santa Ana, LA, Seal Beach, Moreno Valley, OC, RC The LPNLVN Tracheostomy School Nurse assumes responsibility and accountability for the application of the nursing process and the delivery of studentclient care andor contracted services in a school setting. The LPNLVN Tracheostomy School Nurse provides services in accordance with the school district and Amergis' philosophy, standards, policies and physician orders. Essential Duties and Responsibilities: Utilizes the nursing process to plan, implement and evaluate student's care needs Demonstrates on each assignment the provision of skilled nursing care in accordance with prescribed ordersIHP Performs interventions, treatments, and medications administration according to the identified priorities as outlined in the IHP Monitors and documents effectiveness of interventions, treatments, and medications Completes documentation per client expectations Coordinates and facilitates services as requested between the school, local health agencies, and other community resources to meet the needs of the student Communicates with the school district personnel about student's plan of careIHP Notifies District School NurseAmergis SupervisorParentPhysician as outlined in the IHP regarding the following: changes in condition of student and Physician notification to obtain necessary orders Collaborates with schooleducation personnelother health agencies as requested, to design and meet student's health care needs Performs emergency trach change if neededrequired by student's respiratory status Provides trach care to students to maintain the patency of the tube while minimizing risk for infection Performs trach suctioning and cleaning per provider orders Evaluates and assures student safety at all times Adheres to Amergis and school district policies Maintains accuracy in student health record At request of school district participates in student care conferencesIndividualized Education Program (IEP) LPNLVNs should direct questions regarding orders and changes in condition to a Supervising RN (DN andor Clinical Designee) Benefits At Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: Competitive pay & weekly paychecks Health, dental, vision, and life insurance 401(k) savings plan Awards and recognition programs *Benefit eligibility is dependent on employment status.
