Job DescriptionJob DescriptionPlans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. Provides nutritional assessment, education and counseling to individuals andor groups. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.Full time schedule working Monday-Friday from 8am-5pm. Address: 3003 New Hyde Park Road, Suite 409 - Division of EndocrinologyLooking for candidates with vas knowledge in diabetic supplies and use of diabetic pumps.Job Responsibility Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. Assesses nutritional needs and considers patients' cultural, educational, and learning issues. Provides individualized nutrition education to patients, their families, andor significant others based on their needs assessment. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. Communicates patient information to referring providers and appropriate staff in a timely manner. Documents nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided. Performs related duties as required. Duties not mentioned here, but considered related are not essential functions.Job Qualification Bachelor's Degree in Food, Nutrition or Dietetics, required. Registered Dietitian, required. Current Diabetes Educator Certification, required. Knowledge of diabetic supplies and diabetic pumps, preferred. OR Bachelor's Degree in Nursing, required. Current license to practice as a Registered Professional Nurse in New York State, required. Current Diabetes Educator Certification, required. Minimum of three (3) years progressively responsible, related experience to ensure familiarity with scope of program. Knowledge of diabetic supplies and diabetic pumps, preferred.This position will also require to take after hour calls for the physicians after the office closed.*Additional Salary DetailThe salary range andor hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
