Responsible for day-to-day operations for services supported by the IMPACT project within Pediatrics, Behavioral Health, and OBGYN (VIP, Healthy Steps, Reach Out and Read, Social Work, Mental healthand Care Coordination).2. Accountable for and manages development, implementation and ongoing maintenance of facility based projects in support of IMPACT.3. Support staff to create, implement and consolidate workflows.4. Coordinate activities with community partners, scheduling meetings, drafting minutes, orderingsupplies, and other administrative tasks as required by the project.5. Establishes and maintains positive relationships with referral sources both internal and external toNYC H+H, ensuring patients can obtain referral appointments with established referral providers, asneeded.6. Maintains database of patients screened and referred, including getting monthly data forperformance improvement on screening rates, positive rates and responses to positives.7. Ensures adherence to standard work policies and procedures regarding referral processes, including troubleshooting concerns by referral providers and conducting regular communicationon referral workflowsrequirements, for compliance.8. Conducts ongoing curation of our referral resources.9. Completes reports accurately, completely and in a timely way as needed for clinic schedule statistics, productivity and analyses.10. Responsible for data collection for IMPACT, reporting to IMPACT site lead 11. Work closely with IMPACT site lead and Clinical Leads to meet grant objectives and reporting requirements.12. Track progress towards milestones set by IMPACT leadership and communicates project status on a regular basis.13. Perform other duties as assigned by IMPACT site lead - including Dept Clinical Reports and procurement of supplies.14. Responsible for operationalizing workflows for key IMPACT functions, including but not limited to referral pathways, referral tracking, and maintenance of a registry to track high-risk cases.15. Assist with staff schedule and template management.16. Arranges annual staff education, as needed. (i.e. Multistar, coding, etc.).17. Assists staff in maintaining credentialing requirements, as needed.18. Facilitates internal orientation of new staff.19. Ensures processes for patient check in are complete and accurate.20. Handles patient concernscomplaintsfeedback as necessary and appropriate. Ensures patient demographic information is documented accurately and completely with each patient encounter.21. Responsible for ordering office and ERP supplies essential for staff duties timely, completely and accurately.22. Together with clinical site coordinators in other facilities, create processes for tracking IMPACT project metrics and maintain facility database.Minimum Qualifications1. Bachelor's degree from an accredited college or university in Public Health, Public or Business Administration, Social Work, Psychology or Rehabilitation Counseling; and2. Three (3) years of full-time experience in medically-oriented health care and medical support systems environment, two (2) years of which included experience in an administrative or supervisory capacity in such occupations as physical, occupational, inhalation, speech and audiology therapy or as MedicalLaboratory Technologist, Microbiologist (various specialties), Chemist (biochemist), Radiation and X-Ray Technician; or three (3) years of responsible level experience in business management systems or general administration including two (2) years in a supervisory or administrative capacity; or3. Master's degree from an accredited college or university in disciplines, as listed in "1" above; and4. Two (2) years of full-time experience, as described in "2" above, plus one (1) year of experience in an administrative or supervisory capacity in occupations, as listed in "2" above; or5.
