Rutland- Licenced Practical Nurse

Position: Licenced Practical NurseLocation: Brooklyn, NYPay Rate: $34hrSchedule: Evening Shift - 3p-11p OR Night Shift - 10:45p - 7:15a 5x8 -With Every other weekend Responsibilities:- Demonstrates proficient knowledge in the treatment of patients requiring nursing care.- Uses available diagnostic results and other information to make an accurate evaluation.- Applies knowledge of illnesses, injuries, and diseases and uses available resources in performing the nursing process.- Performs timely, comprehensive, and accurate reevaluations and documentation at regular intervals as defined by the department, when a significant change occurs in the resident's condition, and when otherwise warranted.- Is considerate of residents' privacy during the evaluation process.- Contributes to an individualized and appropriate plan of care delineating measurable patient care goals- Contributes to the plan of care making necessary changes as the condition of the patient indicates.- Bases care decisions on resident needs and care priorities.- Monitors patient outcomes.- Monitor continued care needs for discharge and coordinates resources to meet post-institutional requirement.- Collaborate, as appropriate, with Care Manager, physician, and other clinical disciplines in planning interventions to meet resident and family needs in a timely manner.Skills:- Ability to work in surroundings with degree of dust, noise, etc. without undue discomfort- Ability to hear, talk, walk, stand, bend and stretch- Ability to read, write and interpret documents- Ability to sit for long periods of time at desk to perform job functions- Ability to stand for long periods of time at required patient bedside andor procedural areas- Ability to communicate effectively with patients andor co-workers- May be exposed to mechanicalelectricalradiantexplosiveburn and chemical hazards- May be required to handle, dispose of andor identify hazardous waste and hazardous materials- May be required to operate and clean equipment- May be required to handle assist in lifting patients ontoout of exam bedsprocedure stations- Must be trained in and knowledgeable of all KingsbrookRutland policies and procedures pertaining to hazardous waste and hazardous materials and maintain qualifications applicable to assigned work Requirements:- Graduation from an accredited school of practical nursing or completion of equivalent study in an approved program in professional nursing.- Minimum one (1) year clinical experience preferred.- Current NYS LPN Licensure
